Thank you for your purchase of a Coupon for Massage and/or Chiropractic Care from either Today’s Deal or Angie’s List.  We appreciate your business and look forward to seeing you in the office.  Here are some tips and helpful information about how to get the most out of the offer. 

1) New Patient Paperwork.  Everyone who is treated at Chiropractic Care & Sports Rehab, Inc needs to complete the paper work.  Even if you are just coming in for Massage Therapy or Acupuncture we need to know about your health history and current areas of concern.  You can download or print the paperwork by following this link (New Patient Paperwork) or you can fill-out the forms at the time of your appointment.

2) Scheduling your appointment. Please call the office at (314) 439-5548 to make your appointment.  Most likely Elvira, the office manager, will answer the phone.

  • MollieShea, the Massage Therapist is available Monday thru Friday from 9am to 6pm. The latest massage appointment of the day is 5 o’clock.
  • Dr. Spath, the Certified Chiropractic Sports Physician is available Monday thru Friday from 9am to 6pm.  The latest chiropractic appointment of the day is 5:20pm.

3) Expiration Date. The Today’s Deal expires on June 4th 2013. Please call before then to use the Coupon for Massage. The Angie’s List Big Deal expires 180 days after purchase.  Please set-up your visits before this date.

4) Our Location. We are located in the Tempo Medical Building on the corner of Olive Blvd and Tempo Drive. It is about 1/4 mile West of Hwy 270 on the North side of Olive, diagonally across from T.G.I Fridays restaurant.
